👋 नमस्कार दोस्तों!

मैं हूँ Shadab Ansari, Pharmacist & Health Educator, और आप पढ़ रहे हैं “Aapka Pharmacist Dost”।

आज का विषय थोड़ा बड़ा है, लेकिन बेहद जरूरी भी है—हमारे फेफड़े (Lungs)।

हम दिन-रात सांस लेते हैं, लेकिन अक्सर तब तक फेफड़ों के बारे में नहीं सोचते जब तक खांसी, सीने में जकड़न, सांस फूलना, सीटी जैसी आवाज, बलगम या ऑक्सीजन कम होना शुरू न हो जाए।

विश्व स्वास्थ्य संगठन के अनुसार asthma, COPD, lung cancer, pneumonia और tuberculosis दुनिया में lung-related बीमारी और मृत्यु के सबसे बड़े कारणों में शामिल हैं।

और सबसे दिलचस्प बात?

👉 हर सांस की समस्या Asthma नहीं होती।
👉 हर खांसी में antibiotic की जरूरत नहीं होती।
👉 हर nebulization “सांस खोलने” का सही इलाज नहीं है।
👉 और हर inhaler एक जैसा नहीं होता।

आज हम समझेंगे कि किस बीमारी में कौन-सी दवा क्यों दी जाती है और वह शरीर में क्या करती है।

विशेष रूप से inhaled steroids, bronchodilators, antibiotics, anti-TB drugs, anticoagulants, pulmonary hypertension medicines और cancer medicines डॉक्टर की सलाह के अनुसार ही उपयोग किए जाने चाहिए।

खुद से दवा शुरू या बंद न करें।

1️⃣ Lung Diseases को समझने का आसान तरीका

फेफड़ों की बीमारी को broadly इन groups में समझ सकते हैं:

2️⃣ ASTHMA — सांस की नलियों की “अति-संवेदनशीलता”

Asthma में airway में inflammation + bronchoconstriction + mucus के कारण सांस लेने में परेशानी होती है।

WHO के अनुसार 2023 में लगभग 363 million लोग asthma से प्रभावित थे।

सामान्य symptoms

• सांस फूलना

• सीने में जकड़न

• wheezing यानी सीटी जैसी आवाज

• रात/सुबह खांसी

• exercise या धूल/धुएं से symptoms बढ़ना

Asthma में दवाओं की मुख्य classes

🔹 A. Inhaled Corticosteroids — ICS

Common salts:

• Budesonide

• Beclometasone

• Fluticasone

• Mometasone

• Ciclesonide

कैसे काम करते हैं?

ये airway की सूजन और inflammation कम करते हैं।

यही कारण है कि modern asthma management में inhaled corticosteroid-containing therapy बहुत महत्वपूर्ण है। GINA 2026 भी asthma में ICS-containing treatment पर जोर देता है।

Foracort में budesonide + formoterol और Seroflo में fluticasone + salmeterol combination मिलता है।

🔹 B. Short-acting Bronchodilator

Salbutamol

काम: airway के smooth muscles को relax करके bronchial tubes को जल्दी खोलता है।

Brand example

Asthalin — Cipla

Asthalin inhaler में salbutamol होता है और यह rapid bronchodilation के लिए उपयोग किया जाता है।

ध्यान दें

Salbutamol symptom relief देता है, लेकिन underlying airway inflammation का पूरा इलाज नहीं करता।

इसलिए केवल “rescue inhaler” पर निर्भर रहना सही asthma control का substitute नहीं है। GINA 2026 anti-inflammatory reliever approaches पर विशेष जोर देता है।

🔹 C. LABA

Examples:

• Formoterol

• Salmeterol

ये लंबे समय तक airway को relaxed रखने में मदद करते हैं।

अक्सर इन्हें ICS के साथ combination में दिया जाता है।

Example

Budesonide + Formoterol → Foracort / Formonide

🔹 D. Leukotriene Receptor Antagonist

Montelukast

यह leukotriene pathway को block करके airway inflammation और bronchoconstriction को कम करने में मदद करता है।

Representative brand: Montair — Cipla

कुछ patients में allergic rhinitis/asthma phenotype में इसका उपयोग किया जाता है।

3️⃣ COPD — सिर्फ “पुरानी खांसी” नहीं

COPD यानी Chronic Obstructive Pulmonary Disease।

इसके प्रमुख forms में chronic bronchitis और emphysema शामिल हैं।

WHO के अनुसार 2023 में COPD से लगभग 3.4 million deaths हुईं और यह worldwide तीसरा leading cause of death था।

मुख्य symptoms

• लगातार खांसी

• बलगम

• सांस फूलना

• wheezing

• exercise capacity कम होना

• बार-बार chest infection/exacerbation

Smoking इसका बड़ा कारण है, लेकिन household air pollution भी महत्वपूर्ण risk factor है।

COPD में कौन-कौन सी medicines?

🔹 SABA

Salbutamol

Brand:

Asthalin — Cipla

🔹 SAMA

Ipratropium

यह muscarinic receptors को block करके bronchial smooth muscle relaxation करता है।

🔹 SABA + SAMA

Levosalbutamol + Ipratropium

Representative brand:

Duolin — Cipla

Duolin formulations में ipratropium + levosalbutamol combination उपलब्ध है।

🔹 LABA

Examples:

• Formoterol

• Salmeterol

• Indacaterol

• Olodaterol

🔹 LAMA

Examples:

• Tiotropium

• Glycopyrronium

• Umeclidinium

• Aclidinium

इनका काम क्या है?

ये airway के muscarinic-mediated bronchoconstriction को रोककर लंबे समय तक bronchodilation देते हैं।

COPD treatment में inhaled bronchodilators central role रखते हैं।

💡 Pharmacist Point

एक बहुत common गलती है:

“सांस फूल रही है → steroid tablet/injection दे दो।”

हर patient में यह सही approach नहीं है।

COPD का long-term management disease severity, symptoms, exacerbation history और inhaler technique के आधार पर तय किया जाता है।

4️⃣ BRONCHITIS

Bronchitis में bronchial tubes में inflammation होता है।

Acute bronchitis

अक्सर viral infection के कारण होता है।

इसलिए हर acute bronchitis में antibiotic जरूरी नहीं।

Treatment अक्सर supportive होता है:

• hydration

• rest

• fever/pain control

• selected patients में bronchodilator

• underlying cause के अनुसार treatment

अगर bacterial infection clinically suspected हो

Doctor जरूरत के अनुसार antibiotics जैसे:

• Amoxicillin

• Amoxicillin + Clavulanate

• Azithromycin

• Doxycycline

का चयन कर सकते हैं।

Example

Augmentin — Amoxicillin + Clavulanate — GSK

GSK India की prescribing information में Augmentin formulations listed हैं।

5️⃣ PNEUMONIA 🦠

Pneumonia में lung tissue/alveoli में infection और inflammation हो सकता है।

कारण हो सकते हैं:

• Bacteria

• Viruses

• Fungi

• Aspiration

Symptoms

• fever

• cough

• sputum

• chest pain

• breathlessness

• weakness

• कभी-कभी low oxygen

Pneumonia की medicines

Treatment organism, severity, age, comorbidities और local resistance pattern पर निर्भर करता है।

Possible antibiotics में शामिल हो सकते हैं:

🔹 Amoxicillin

Class: Penicillin

Mechanism: bacterial cell-wall synthesis को inhibit करता है।

🔹 Amoxicillin + Clavulanate

Clavulanate beta-lactamase enzymes को inhibit करके कुछ resistant bacteria के खिलाफ amoxicillin की activity बढ़ाता है।

Brand: Augmentin — GSK

🔹 Azithromycin

Class: Macrolide

Mechanism: bacterial 50S ribosomal subunit पर bind होकर protein synthesis को inhibit करता है।

Brand examples:

• Zithromax — original brand marketed by different entities depending on market

• Azicip — Cipla

• Azimax — Cipla

Indian market listings में azithromycin के कई brands उपलब्ध हैं।

⚠️ Important

Antibiotic का चयन “खांसी है इसलिए antibiotic” के आधार पर नहीं होना चाहिए।

गलत antibiotic = resistance + unnecessary side effects + treatment failure.

6️⃣ TUBERCULOSIS — भारत में सबसे महत्वपूर्ण lung infections में से एक

TB का कारण Mycobacterium tuberculosis है।

Typical symptoms

• 2 सप्ताह या उससे अधिक की खांसी

• वजन कम होना

• रात में पसीना

• fever

• कमजोरी

• कभी blood in sputum

WHO के अनुसार TB diagnosis में rapid molecular tests और drug-resistance testing महत्वपूर्ण हैं।

TB की मुख्य medicines

Traditional first-line drugs:

1. Rifampicin

RNA polymerase को inhibit करता है।

2. Isoniazid

Mycobacterial cell-wall mycolic acid synthesis को inhibit करता है।

3. Pyrazinamide

Intracellular/acidic environment में TB bacilli पर प्रभाव डालता है।

4. Ethambutol

Mycobacterial cell-wall components की synthesis को inhibit करता है।

WHO के अनुसार drug-susceptible TB में rifampicin, isoniazid, pyrazinamide और ethambutol प्रमुख medicines हैं।

🚨 TB का सबसे बड़ा lesson

TB की दवा बीच में बंद करना खतरनाक हो सकता है।

Incomplete treatment drug resistance को बढ़ावा दे सकता है। WHO के अनुसार drug-resistant TB के लिए अलग और अधिक complex regimens की जरूरत होती है।

2025 WHO recommendations में eligible MDR/RR-TB patients के लिए newer 6-month all-oral regimens जैसे BDLLfxC को भी शामिल किया गया है।

7️⃣ BRONCHIECTASIS

इसमें bronchi स्थायी रूप से dilate/damaged हो सकते हैं।

Symptoms

• लंबे समय से cough

• ज्यादा sputum

• recurrent chest infections

• breathlessness

• कभी hemoptysis

Management

• Airway clearance techniques

• Physiotherapy

• Hydration

• Infection का appropriate antibiotic treatment

• Bronchodilators selected patients में

• Underlying cause का treatment

हर patient को routine antibiotic या steroid देना सही नहीं है।

8️⃣ INTERSTITIAL LUNG DISEASE — ILD

ILD एक single disease नहीं बल्कि कई disorders का group है।

इसमें lung tissue में inflammation या fibrosis हो सकती है।

Symptoms

• progressive breathlessness

• dry cough

• reduced exercise tolerance

• कभी clubbing

9️⃣ IDIOPATHIC PULMONARY FIBROSIS — IPF

IPF में lung tissue धीरे-धीरे scar/fibrotic हो सकता है।

दो महत्वपूर्ण antifibrotic medicines:

🔹 Pirfenidone

Mechanism: fibrotic pathways और inflammatory processes को modulate करके fibrosis progression को धीमा करने में मदद करता है।

Indian brand examples

• Pirfetab — Zydus

• Fiboresp — Glenmark

• Pirfenair — Dr. Reddy's

Indian drug listings में pirfenidone के कई brands उपलब्ध हैं।

🔹 Nintedanib

यह multi-target tyrosine kinase inhibitor है और abnormal fibrotic signaling को suppress करने में मदद करता है।

Boehringer Ingelheim के product portfolio में Ofev/nintedanib को IPF और progressive fibrosing ILD के लिए listed किया गया है।

🔟 PULMONARY HYPERTENSION

यह सामान्य “BP बढ़ना” नहीं है।

इसमें pulmonary arteries में pressure बढ़ता है।

Symptoms

• exertional breathlessness

• fatigue

• chest discomfort

• dizziness/syncope

• edema

प्रमुख medicines

Sildenafil

Mechanism: PDE-5 inhibition → cGMP pathway बढ़ता है → pulmonary vascular smooth muscle relaxation।

Brand: Revatio

Revatio specifically pulmonary arterial hypertension के treatment के लिए indicated है।

Bosentan

यह endothelin receptor antagonist है।

Brand: Tracleer

Pulmonary arterial hypertension में exercise capacity और symptoms improve करने के लिए इसका उपयोग किया गया है।

अन्य modern therapies

इनका चयन pulmonary hypertension के specific WHO group, severity और hemodynamic findings के आधार पर specialist करता है।

1️⃣1️⃣ PULMONARY EMBOLISM — PE 🩸

Pulmonary embolism में blood clot pulmonary artery को block कर सकता है।

Warning symptoms

🚨 अचानक सांस फूलना
🚨 chest pain
🚨 तेज heartbeat
🚨 coughing blood
🚨 collapse/syncope

यह emergency हो सकती है।

PE की medicines

Anticoagulants

• Heparin

• LMWH

• Apixaban

• Rivaroxaban

Apixaban

Mechanism: direct Factor Xa inhibitor.

Brand: Eliquis — Pfizer

Pfizer India की prescribing information के अनुसार apixaban DVT और pulmonary embolism के treatment तथा recurrent DVT/PE prevention में indicated है।

Severe PE

Selected unstable patients में thrombolysis या catheter/surgical intervention की आवश्यकता हो सकती है।

1️⃣2️⃣ LUNG CANCER 🎗️

Lung cancer broadly:

🔹 NSCLC

Non-Small Cell Lung Cancer

🔹 SCLC

Small Cell Lung Cancer

Treatment सिर्फ “एक cancer tablet” से नहीं होता।

यह depend करता है:

• stage

• histology

• molecular mutation

• PD-L1

• overall health

• metastatic disease

पर।

NCI के अनुसार NSCLC में surgery, chemotherapy, radiation, targeted therapy और immunotherapy सभी की भूमिका हो सकती है।

Immunotherapy

Important drugs:

इनका काम cancer cells द्वारा immune system से बचने की क्षमता को कम करके immune response को मजबूत करना है।

NCI के अनुसार immunotherapy NSCLC treatment में महत्वपूर्ण भूमिका निभाती है और इसका उपयोग disease stage तथा biomarker profile के अनुसार किया जाता है।

Small Cell Lung Cancer

SCLC में chemotherapy + immunotherapy का combination कई situations में उपयोग किया जाता है।

Examples:

• Atezolizumab

• Durvalumab

• Platinum chemotherapy

• Etoposide

NCI के अनुसार extensive-stage SCLC में atezolizumab या durvalumab को combination chemotherapy के साथ इस्तेमाल किया जा सकता है।

1️⃣3️⃣ PLEURAL EFFUSION

Pleural cavity में fluid जमा होना।

यह खुद कोई एक बीमारी नहीं, बल्कि कई diseases का परिणाम हो सकता है:

• heart failure

• pneumonia

• TB

• cancer

• liver/kidney disease

Treatment

Cause के अनुसार:

• Diuretics

• Antibiotics

• Anti-TB therapy

• Cancer treatment

• Therapeutic thoracentesis

• Chest tube drainage

इसलिए केवल “पानी की गोली” देकर हर pleural effusion का इलाज नहीं किया जा सकता।

1️⃣4️⃣ PNEUMOTHORAX

Pleural space में air जमा होने से lung partially या completely collapse हो सकता है।

Symptoms

• अचानक chest pain

• अचानक breathlessness

• reduced breath sounds

Management

Small stable pneumothorax में observation हो सकता है।

लेकिन बड़ा या symptomatic pneumothorax में:

➡️ aspiration
➡️ chest tube
➡️ selected cases में surgery

की जरूरत हो सकती है।

⚠️ Tension pneumothorax

यह life-threatening emergency है और immediate decompression की आवश्यकता हो सकती है।

1️⃣5️⃣ ARDS — Acute Respiratory Distress Syndrome

ARDS में lungs में severe inflammation और oxygenation failure हो सकता है।

Causes:

• severe pneumonia

• sepsis

• aspiration

• major trauma

• pancreatitis आदि

Management

यह ICU-level condition है।

मुख्य strategies:

• Oxygen support

• Lung-protective mechanical ventilation

• Appropriate PEEP

• Prone positioning in selected severe cases

• Underlying cause का treatment

• Fluid strategy

• Selected patients में neuromuscular blockade/other advanced ICU approaches

यह ऐसी स्थिति नहीं है जिसे घर पर cough syrup या nebulizer से manage किया जा सके।

1️⃣6️⃣ OCCUPATIONAL LUNG DISEASES

कुछ jobs में लंबे समय तक dust/fumes/chemicals के exposure से lung disease हो सकती है।

Examples:

• Silicosis

• Asbestosis

• Coal worker pneumoconiosis

• Occupational asthma

Management का पहला rule:

Exposure control.

Medicine से ज्यादा जरूरी हो सकता है:

• workplace protection

• respirator/PPE

• exposure removal

• smoking cessation

• vaccination

• pulmonary rehabilitation

• disease-specific treatment

🧠 एक ही “सांस फूलने” के पीछे इतनी अलग-अलग बीमारियाँ क्यों?

यही lung medicine की सबसे interesting बात है।

Dust exposure के बाद wheezing → asthma हो सकता है।

40 pack-year smoking history + chronic sputum → COPD हो सकता है।

Patient C

2–3 weeks cough + weight loss + night sweats → TB evaluation जरूरी हो सकती है।

Patient D

Sudden breathlessness + chest pain → pulmonary embolism जैसी emergency हो सकती है।

Patient E

Slowly progressive breathlessness + dry cough → ILD/IPF consider हो सकता है।

Patient F

Long-term cough + hemoptysis + weight loss → lung cancer सहित कई causes evaluate करने पड़ सकते हैं।

इसलिए केवल symptom देखकर medicine चुनना unsafe हो सकता है।

🩺 LUNG DISEASE MANAGEMENT — केवल Medicine नहीं

Lung disease management के 6 pillars समझिए:

1️⃣ सही Diagnosis

• Clinical history

• Physical examination

• Pulse oximetry

• Chest X-ray

• CT chest

• Spirometry

• Blood tests

• Sputum examination

• Microbiology

• TB molecular testing

• Cancer biopsy/molecular testing

Disease के अनुसार tests अलग होते हैं।

2️⃣ Smoking बंद करना 🚭

COPD और lung cancer prevention में सबसे powerful interventions में से एक।

3️⃣ Air Pollution Control

WHO के अनुसार air pollution respiratory health को खराब कर सकता है और lung function decline में योगदान दे सकता है।

भारत जैसे देश में विशेष रूप से ध्यान दें:

• traffic pollution

• biomass smoke

• indoor smoke

• occupational dust

• construction dust

4️⃣ Vaccination 💉

Selected patients में vaccination respiratory infection और exacerbation risk कम करने में मदद कर सकती है।

Relevant vaccines disease/age/risk के अनुसार:

• Influenza

• Pneumococcal

• COVID-19

• Pertussis आदि

5️⃣ Pulmonary Rehabilitation

विशेषकर chronic lung diseases में:

• exercise training

• breathing techniques

• education

• nutrition

• activity planning

quality of life improve कर सकते हैं।

WHO भी COPD management में pulmonary rehabilitation और physical activity को महत्वपूर्ण मानता है।

6️⃣ सही Inhaler Technique

यह point बहुत underrated है।

कई बार patient कहता है:

“Inhaler से फायदा नहीं हो रहा।”

लेकिन समस्या medicine नहीं बल्कि गलत inhaler technique हो सकती है।

💨 Inhaler के बाद Mouth Rinse क्यों?

यदि inhaled corticosteroid use किया गया है तो mouth rinse करने से:

• oral candidiasis का risk कम करने में मदद

• throat irritation/hoarseness कम करने में मदद

हो सकती है।

Foracort जैसी ICS-containing therapy के साथ mouth rinsing की सलाह दी जाती है।

❌ Myths vs Facts

Myth 1:

“Inhaler की आदत लग जाती है।”

Fact:

Inhaler delivery device है। Medicine की dependence को “नशे की आदत” समझना गलत है।

Myth 2:

“Inhaler आखिरी stage की medicine है।”

Fact:

Asthma में inhaled medicines disease management की foundation हो सकती हैं।

Myth 3:

कई respiratory infections viral होते हैं।

Myth 4:

“Steroid हमेशा dangerous है।”

Fact:

Steroids की safety drug, dose, route और duration पर निर्भर करती है। Inhaled corticosteroids और systemic corticosteroids को एक जैसा नहीं समझना चाहिए।

Myth 5:

“TB की दवा कुछ दिन खाकर आराम हो जाए तो बंद कर सकते हैं।”

Fact:

बिल्कुल नहीं। Treatment interruption resistance और treatment failure का risk बढ़ा सकता है।

🤯 कुछ Interesting Lung Facts

Fact 1

हमारे lungs में लगभग 300 million alveoli होते हैं।

Fact 2

Right lung सामान्यतः left lung से बड़ा होता है क्योंकि heart left side में space लेता है।

Fact 3

हमारा lung केवल oxygen लेने का organ नहीं है—यह gas exchange के अलावा कई metabolic और vascular functions में भी भाग लेता है।

Fact 4

Asthma में समस्या केवल “airway tightness” नहीं बल्कि airway inflammation भी है।

Fact 5

COPD को केवल “smoker's cough” समझना dangerous हो सकता है।

Fact 6

Lung cancer treatment तेजी से personalized medicine की तरफ बढ़ रहा है—अब mutation और biomarker testing treatment selection में महत्वपूर्ण हैं।

TB के लिए 2025 WHO recommendations में shorter all-oral drug-resistant TB regimens ने treatment landscape को काफी बदला है।

🚨 कब तुरंत Hospital जाना चाहिए?

अगर patient में:

🔴 अचानक बहुत ज्यादा सांस फूलना
🔴 SpO₂ लगातार कम होना
🔴 blue lips/fingertips
🔴 severe chest pain
🔴 coughing blood
🔴 confusion
🔴 बेहोशी
🔴 severe asthma attack
🔴 suspected tension pneumothorax
🔴 rapidly worsening respiratory distress

हो तो self-medication नहीं—urgent medical evaluation जरूरी है।

❓ FAQs

Q1. क्या हर सांस फूलने वाले patient को Salbutamol दे सकते हैं?

नहीं। Breathlessness के कई कारण हो सकते हैं—Asthma, COPD, pneumonia, PE, heart failure, anemia, anxiety आदि।

Q2. क्या nebulizer inhaler से ज्यादा powerful होता है?

जरूरी नहीं।

सही patient में सही inhaler technique बहुत प्रभावी हो सकती है। Nebulizer की जरूरत clinical situation और patient factors पर निर्भर करती है।

Q3. Asthma और COPD में सबसे बड़ा difference क्या है?

Asthma में airway obstruction अक्सर variable/reversible होता है, जबकि COPD में persistent airflow limitation और progressive disease हो सकती है।

Q4. क्या antibiotics cough को तुरंत ठीक कर देती हैं?

नहीं। Antibiotics केवल susceptible bacterial infections में उपयोगी हैं।

Q5. TB की दवा और सामान्य antibiotic में क्या difference है?

TB के लिए specific anti-tuberculosis regimen चाहिए। सामान्य antibiotic लेकर TB का उचित इलाज नहीं किया जा सकता।

Q6. क्या oxygen हर सांस के patient को देना चाहिए?

नहीं।

Oxygen भी एक medical treatment है और unnecessary/high-concentration oxygen कुछ conditions में harmful हो सकता है। Target saturation patient की condition के अनुसार clinician तय करता है।

Q7. क्या lung cancer का इलाज संभव है?

कई lung cancers में cure संभव है, विशेषकर जब disease early stage में detect हो। Advanced disease में भी surgery, chemotherapy, targeted therapy और immunotherapy से disease control और survival improve किया जा सकता है।

👨‍⚕️ Pharmacist की खास सलाह

एक pharmacist के तौर पर मेरी सबसे important advice है:

💡 “Salt को पहचानिए, सिर्फ Brand को नहीं।”

अगर prescription में लिखा है:

Budesonide + Formoterol

तो केवल brand देखकर confused होने की जरूरत नहीं।

Salt आपको बताता है कि medicine क्या है और किस pharmacological class में है।

लेकिन इसका मतलब यह नहीं कि हर brand/formulation interchangeable है—strength, device और indication verify करना जरूरी है।

💨 दूसरा Golden Rule:

“Inhaler की technique भी dose का हिस्सा है।”

Medicine सही हो लेकिन patient:

❌ inhaler shake न करे
❌ सही समय पर inhale न करे
❌ breath hold न करे
❌ device गलत तरीके से use करे

तो drug delivery कम हो सकती है।

इसलिए pharmacist को सिर्फ medicine देना नहीं बल्कि device counselling भी करनी चाहिए।

🧩 तीसरा Golden Rule:

“Antibiotic ≠ हर respiratory disease की medicine”

Asthma → anti-inflammatory + bronchodilator strategy
COPD → bronchodilator-centered strategy
TB → specific anti-TB regimen
Pneumonia → cause/severity-based treatment
IPF → antifibrotic therapy in appropriate patients
PE → anticoagulation/thrombus management
Lung cancer → stage + histology + biomarkers आधारित therapy

यही difference समझना एक अच्छे healthcare professional की पहचान है।

📌 Real-Life Example

मान लीजिए 55 साल का व्यक्ति pharmacy पर आता है और कहता है:

“भाई साहब, कई दिनों से सांस फूल रही है और खांसी भी है। कोई अच्छा inhaler दे दीजिए।”

यहां केवल inhaler देना सही pharmacy practice नहीं होगी।

पहले पूछना चाहिए:

• सांस कब से फूल रही है?

• अचानक या धीरे-धीरे?

• wheezing है?

• fever है?

• sputum है?

• blood तो नहीं?

• smoking history?

• पहले asthma/COPD?

• inhaler पहले इस्तेमाल किया?

• chest pain?

• oxygen saturation उपलब्ध है?

क्योंकि यही symptoms asthma से लेकर pneumonia और pulmonary embolism तक कई conditions में दिखाई दे सकते हैं।

📚 सबसे बड़ा Takeaway

फेफड़ों की दवाओं को याद रखने का सबसे आसान तरीका है:

हम अक्सर lungs को तब याद करते हैं जब सांस लेने में तकलीफ होती है।

लेकिन healthy lungs के लिए prevention उतना ही महत्वपूर्ण है जितना treatment।
