एक ही “खुजली वाली क्रीम” हर बीमारी के लिए क्यों सही नहीं होती? जानिए Skin Diseases को पहचानने और सही उपचार समझने की पूरी जानकारी

आज का विषय है Skin Diseases यानी त्वचा से जुड़ी बीमारियाँ। त्वचा हमारे शरीर का सबसे बड़ा अंग है और यही हमें बाहरी संक्रमण, धूल-मिट्टी, रसायनों और वातावरण से बचाने में महत्वपूर्ण भूमिका निभाती है।

लेकिन एक परेशानी यह है कि त्वचा पर दिखने वाले कई लक्षण एक जैसे लग सकते हैं। खुजली, लाल चकत्ते, दाने, पपड़ी, जलन या गोल निशान देखकर लोग अक्सर तुरंत कोई cream खरीद लेते हैं।

यहीं से समस्या शुरू हो सकती है।

क्योंकि fungal infection, eczema, psoriasis, allergy और bacterial infection का इलाज एक जैसा नहीं होता। भारत में त्वचा रोगों के गलत उपचार और mismanagement को देखते हुए ICMR ने acne/rosacea, alopecia, bacterial infections, dermatophytosis, eczema/dermatitis, psoriasis, scabies, herpes/varicella, vitiligo और urticaria जैसी कई स्थितियों के लिए Standard Treatment Workflows विकसित किए हैं।

Skin Disease कोई एक बीमारी नहीं है, बल्कि त्वचा को प्रभावित करने वाली बहुत बड़ी संख्या में conditions का समूह है।

इनके कारण हो सकते हैं:

WHO की 2026 की practical handbook भी बताती है कि कई skin conditions के पीछे bacteria, fungi, parasites, viruses और अन्य कारण हो सकते हैं और कुछ skin manifestations neglected tropical diseases से भी संबंधित हो सकती हैं।

2. Skin Diseases की प्रमुख Categories

त्वचा की बीमारियों को मोटे तौर पर इस तरह समझ सकते हैं:

ICMR के Indian skin-disease workflows में भी acne/rosacea, alopecia, bacterial infections, drug reactions, dermatophytosis, eczema/dermatitis, immunobullous diseases, psoriasis, scabies, varicella/herpes, vitiligo और urticaria को प्रमुख clinical modules के रूप में शामिल किया गया है।

3. Skin Disease के Common Symptoms

त्वचा की बीमारी में निम्न लक्षण दिखाई दे सकते हैं:

• खुजली
• लालिमा
• सूजन
• छोटे या बड़े दाने
• गोल-गोल चकत्ते
• पपड़ी बनना
• त्वचा का सूखना
• जलन
• दर्द
• फफोले
• पस
• त्वचा का रंग बदलना
• घाव
• बाल झड़ना
• नाखून मोटे या टूटने लगना

लेकिन सिर्फ symptom देखकर diagnosis करना हमेशा संभव नहीं होता।

उदाहरण के लिए, खुजली fungal infection में भी हो सकती है और eczema या scabies में भी।

4. कौन सी Skin Disease Contagious होती है?

सभी skin diseases एक व्यक्ति से दूसरे व्यक्ति में नहीं फैलतीं।

फैल सकती हैं:

• Scabies
• Ringworm/fungal infections
• Impetigo
• कुछ viral infections
• Warts
• कुछ parasitic infections

आमतौर पर contagious नहीं:

• Psoriasis
• Vitiligo
• Atopic dermatitis
• Acne
• Melasma
• अधिकांश autoimmune skin diseases

इसलिए psoriasis या vitiligo वाले व्यक्ति से दूरी बनाना गलत और stigmatizing है।

5. Skin Diseases में इस्तेमाल होने वाली प्रमुख Medicines

यहाँ एक educational overview दिया जा रहा है। किसी patient के लिए कौन-सी medicine सही है, यह diagnosis, severity, age, location और अन्य factors पर निर्भर करता है।

6. Important Medicines — थोड़ा Detail में

🧴 1. Clotrimazole

Class: Azole antifungal

कहाँ उपयोग होता है?

• Ringworm
• Tinea corporis
• Tinea cruris
• कुछ superficial candidal infections

कैसे काम करता है?

यह fungus की cell membrane बनाने वाली प्रक्रिया को प्रभावित करता है, जिससे fungus की growth रुकती है।

Route

मुख्यतः topical।

Important point

हर गोल दाग को fungal infection मान लेना सही नहीं है।

भारत में dermatophytosis के treatment में resistance और treatment failure की समस्या भी reported है, इसलिए बार-बार खुद से antifungal बदलना सही approach नहीं है।

🧴 2. Terbinafine

Class: Allylamine antifungal

यह fungal cell membrane में आवश्यक ergosterol synthesis को प्रभावित करता है।

यह dermatophyte infections में उपयोगी antifungal है।

लेकिन oral terbinafine जैसी systemic therapy डॉक्टर की supervision में ही होनी चाहिए, क्योंकि patient की medical history और drug safety considerations महत्वपूर्ण होते हैं।

🧴 3. Adapalene

Class: Topical retinoid

मुख्यतः acne में इस्तेमाल होता है।

कैसे काम करता है?

यह skin cell turnover और follicular plugging को नियंत्रित करने में मदद करता है, जिससे pores बंद होने की tendency कम होती है।

Galderma के अनुसार adapalene 0.1% और 0.3% topical formulations उपलब्ध हैं; adapalene + benzoyl peroxide combination acne के कई mechanisms को target करता है।

Practical point

शुरुआत में dryness, irritation या हल्की जलन हो सकती है। इसलिए “ज्यादा लगाऊँगा तो जल्दी ठीक होगा” वाला नियम यहाँ लागू नहीं होता।

7. Acne का Treatment कैसे समझें?

Acne केवल “गंदे चेहरे” की समस्या नहीं है।

इसमें follicular plugging, sebum, inflammation और microbial factors की भूमिका होती है।

भारत में 2025 के PRACT-India consensus ने acne management में topical retinoids की महत्वपूर्ण भूमिका और benzoyl peroxide जैसे agents के साथ rational combination therapy पर जोर दिया है।

Common treatment options

• Adapalene
• Benzoyl peroxide
• Adapalene + benzoyl peroxide
• Azelaic acid
• Selected topical antibiotics
• कुछ patients में oral antibiotics
• Hormonal treatment — selected females
• Isotretinoin — severe/refractory acne में specialist supervision

Oral isotretinoin को self-medication में बिल्कुल नहीं लेना चाहिए।

8. Scabies — सिर्फ खुजली नहीं!

Scabies एक mite infestation है।

इसमें अक्सर:

• रात में ज्यादा खुजली
• उंगलियों के बीच lesions
• wrist
• waist
• genital area
• axilla आदि प्रभावित हो सकते हैं।

प्रमुख treatment

Permethrin 5% एक commonly used scabicide है। AAD के अनुसार scabies treatment में diagnosed patient के साथ close contacts को भी treatment की आवश्यकता हो सकती है, ताकि reinfection और outbreaks को रोका जा सके।

सबसे बड़ी गलती

सिर्फ patient cream लगा ले और घर के बाकी close contacts untreated रहें—तो infection दोबारा हो सकता है।

9. Eczema / Atopic Dermatitis

Atopic dermatitis एक chronic inflammatory skin disease है जिसमें:

• खुजली
• dryness
• redness
• recurrent flare-ups

हो सकते हैं।

Management केवल steroid cream तक सीमित नहीं है।

इसमें:

🧴 नियमित moisturization
🚿 उचित bathing habits
🧼 irritants से बचाव
💊 जरूरत के अनुसार topical medicines
👨‍⚕️ flare control
📅 long-term follow-up

महत्वपूर्ण हैं।

2026 में प्रकाशित भारतीय expert guidelines ने atopic dermatitis में moisturisers, topical corticosteroids, calcineurin inhibitors और selected newer systemic/biologic therapies के individualized use पर जोर दिया है।

10. Psoriasis

Psoriasis एक chronic immune-mediated inflammatory disease है।

इसमें अक्सर:

• लाल plaques
• सफेद/चांदी जैसी scaling
• scalp lesions
• elbows/knees involvement

देखे जा सकते हैं।

कुछ patients में psoriatic arthritis भी विकसित हो सकती है।

Treatment severity और body site के अनुसार बदलता है।

Possible treatment categories

• Moisturizers
• Topical corticosteroids
• Vitamin-D analogues
• Topical calcineurin inhibitors — selected areas
• Phototherapy
• Oral systemic medicines
• Biologic therapies

AAD के अनुसार psoriasis में corticosteroids, vitamin-D analogues और selected delicate areas में topical calcineurin inhibitors जैसे विकल्प इस्तेमाल किए जाते हैं।

11. Brand Name + Company Examples 🇮🇳

महत्वपूर्ण: Brand availability, strength और formulation समय तथा location के अनुसार बदल सकती है। नीचे केवल verified examples दिए जा रहे हैं—ये किसी patient के लिए recommendation नहीं हैं।

Clotrimazole

• Candid Cream — Glenmark Pharmaceuticals
• Candid range में cream, lotion, powder आदि formulations उपलब्ध हैं। Glenmark की current product information में Candid को dermatology/antifungal portfolio में शामिल किया गया है।

Tacrolimus

• Takfa — Intas Pharmaceuticals
• 0.03% और 0.1% topical strengths Intas की product listing में दर्ज हैं।

Ciclopirox

• Nailrox — Intas Pharmaceuticals
• 1% और 8% formulations Intas की product listing में मौजूद हैं।

Acitretin

• Actoid — Intas Pharmaceuticals
• 10 mg और 25 mg strengths listed हैं। यह prescription-level systemic retinoid है और specialist supervision जरूरी है।

Azelaic Acid

• Ezanic — Intas Pharmaceuticals
• 10% और 20% formulations listed हैं।

Tofacitinib

• Tofatas — Intas Pharmaceuticals
• यह systemic immunomodulatory therapy है और routine self-medication के लिए नहीं है।

Adapalene / Acne

• Differin — Galderma

Galderma की official product information adapalene तथा adapalene + benzoyl peroxide formulations को acne treatment portfolio में सूचीबद्ध करती है।

ध्यान रखें: Brand का नाम जानना पर्याप्त नहीं है। हमेशा salt, strength, formulation और indication भी देखें।

12. सबसे जरूरी बात: “Steroid वाली Cream” से सावधान! ⚠️

Skin practice में एक common problem है कि लोग ऐसी combination creams इस्तेमाल करने लगते हैं जिनमें:

steroid + antifungal + antibiotic जैसे multiple ingredients होते हैं।

शुरुआत में लालिमा और खुजली कम हो सकती है, इसलिए patient को लगता है:

“Cream बहुत बढ़िया है।”

लेकिन underlying disease सही तरह treat न होने पर समस्या छिप सकती है या बिगड़ सकती है।

ICMR ने skin-disease workflows में rational use of topical corticosteroids के लिए अलग module रखा है—यह अपने आप में बताता है कि topical steroid का सही उपयोग कितना महत्वपूर्ण है।

इसलिए:

❌ हर खुजली में steroid नहीं
❌ हर fungal infection में steroid combination नहीं
❌ चेहरे पर potent steroid खुद से नहीं
❌ लंबे समय तक steroid बिना medical supervision नहीं

13. Skin Diseases का सही Management

सिर्फ medicine लगाने से हमेशा skin health ठीक नहीं होती।

1️⃣ सही Diagnosis

सबसे पहला कदम है:

“असल बीमारी क्या है?”

2️⃣ सही Medicine

Fungal infection → antifungal
Bacterial infection → selected antibiotic
Scabies → scabicide
Eczema → anti-inflammatory + skin barrier care
Acne → acne-specific therapy
Psoriasis → severity-based treatment

3️⃣ Skin Care

• Gentle cleanser
• जरूरत के अनुसार moisturizer
• Excessive scrubbing से बचें
• बहुत गर्म पानी से बचें
• साफ towel इस्तेमाल करें

4️⃣ Hygiene

खासकर infectious conditions में:

• towel share न करें
• कपड़े नियमित बदलें
• affected area को उचित रूप से dry रखें

5️⃣ Follow-up

अगर treatment के बावजूद बीमारी लगातार बढ़ रही है तो diagnosis दोबारा evaluate करना पड़ सकता है।

14. क्या करें और क्या न करें?

• Medicine का पूरा course/निर्धारित duration follow करें
• Prescribed cream सही मात्रा और तरीके से लगाएं
• Skin को साफ और dry रखें

• दूसरे व्यक्ति की prescription copy न करें
• हर rash को fungal infection न मानें
• बिना सलाह steroid cream न लगाएं
• Antibiotic का unnecessary use न करें
• Antifungal बार-बार बदलते न रहें
• एक साथ कई creams न मिलाएं
• बीमारी ठीक होते ही prescribed treatment खुद से बंद न करें

15. Common Mistakes जो Pharmacy पर अक्सर देखने को मिलती हैं

❌ गलती 1: “दाद है, कोई भी antifungal दे दीजिए।”

हर गोल या खुजली वाला rash fungal नहीं होता।

❌ गलती 2: “Steroid वाली cream सबसे जल्दी आराम देती है।”

Temporary symptom relief और disease cure एक चीज नहीं हैं।

❌ गलती 3: “Antibiotic cream लगा लो, infection है।”

हर skin infection bacterial नहीं होता।

❌ गलती 4: “Cream ज्यादा लगाओगे तो जल्दी ठीक होगा।”

दवा की मात्रा बढ़ाने से effectiveness हमेशा नहीं बढ़ती; irritation और adverse effects बढ़ सकते हैं।

✅ Fact:

Treatment response disease, location, severity और drug पर निर्भर करता है।

❌ Myth:

“Vitiligo छूने से फैलता है।”

✅ Fact:

Vitiligo contagious नहीं है।

❌ Myth:

“Acne सिर्फ teenagers को होता है।”

✅ Fact:

Adults में भी acne हो सकता है।

❌ Myth:

“महंगी cream हमेशा बेहतर होती है।”

✅ Fact:

सही medicine का चुनाव diagnosis और evidence पर होना चाहिए, केवल price पर नहीं।

“Natural है तो हमेशा safe है।”

एक ही symptom—जैसे खुजली—कई अलग diseases में दिखाई दे सकता है।

Scabies में केवल patient का treatment पर्याप्त नहीं हो सकता; close contacts का management भी reinfection रोकने में महत्वपूर्ण है।

🔹 Fact 4

Acne का treatment केवल existing pimples हटाने के लिए नहीं, बल्कि new lesions और scarring रोकने के लिए भी किया जाता है।

🔹 Fact 5

भारत में dermatophytosis के treatment में difficult-to-treat और reduced-susceptibility cases की चिंता सामने आई है।

🔹 Fact 6

Atopic dermatitis का treatment केवल “खुजली की cream” नहीं है—skin-barrier care भी treatment का महत्वपूर्ण हिस्सा है।

18. 🚨 Red Flags — कब तुरंत Doctor को दिखाएं?

इन स्थितियों में self-treatment करने की बजाय medical evaluation जरूरी है:

🚨 सांस लेने में परेशानी के साथ rash
🚨 चेहरे/होंठ/आंखों के आसपास तेजी से swelling
🚨 बहुत तेजी से फैलता rash
🚨 high fever के साथ rash
🚨 पूरे शरीर पर blisters
🚨 आंख या मुंह के अंदर lesions

🚨 तेजी से फैलता bacterial infection
🚨 त्वचा का काला/बैंगनी पड़ना

19. Patient Counselling — Pharmacist कैसे समझाए? 👨‍⚕️

अगर कोई patient pharmacy पर कहता है:

“भाई साहब, खुजली है, कोई अच्छी cream दे दीजिए।”

तो केवल cream देना हमेशा सही approach नहीं है।

पहले पूछना चाहिए:

• खुजली कब से है?
• कहाँ है?
• गोल निशान है या नहीं?
• रात में ज्यादा होती है?
• घर में किसी और को भी है?
• कोई नई medicine शुरू की?
• कोई नई cream/soap लगाया?
• पस या घाव है?
• पहले क्या treatment लिया?
• कोई steroid combination इस्तेमाल किया?

यही छोटा-सा history taking गलत medicine देने से बचा सकता है।

20. Real-Life Example 👤

यह एक काल्पनिक educational example है।

मान लीजिए राहुल को जांघ के पास गोल-गोल खुजली वाले patches हुए।

उसने pharmacy से बिना diagnosis के एक ऐसी combination cream ले ली जिसमें antifungal के साथ steroid भी था।

कुछ दिनों में redness और itching कम हो गई।

राहुल ने सोचा:

“बीमारी ठीक हो गई।”

लेकिन कुछ समय बाद lesion फिर आया और पहले से ज्यादा फैल गया।

गलती कहाँ हुई?

समस्या यह थी कि symptom कम होना और infection का पूरी तरह खत्म होना एक ही बात नहीं है।

साथ ही, steroid component ने clinical appearance को बदल दिया हो सकता है।

सही approach क्या होती?

👉 Proper clinical assessment
👉 जरूरत पड़ने पर testing
👉 सही antifungal regimen
👉 उचित duration
👉 recurrence के कारणों की पहचान

यही कारण है कि “कोई भी खुजली = कोई भी cream” वाला तरीका सुरक्षित नहीं है।

21. आपका फार्मासिस्ट दोस्त की सलाह 💊

Skin disease में मेरी सबसे महत्वपूर्ण सलाह है:

पहले Diagnosis → फिर Medicine → फिर सही Duration → फिर Follow-up

किसी भी skin problem में सिर्फ brand name याद रखना पर्याप्त नहीं है।

आपको कम से कम यह पता होना चाहिए:

Salt क्या है?
किस class की medicine है?
किस condition में उपयोग होती है?
कितने समय के लिए दी गई है?
क्या कोई contraindication है?
क्या दूसरी medicines के साथ interaction हो सकता है?

और सबसे जरूरी—

⚠️ Steroid को “हर बीमारी की cream” न समझें।

Antibiotic, antifungal और antiviral medicines भी जहाँ जरूरी हों, वहीं और उचित तरीके से इस्तेमाल की जानी चाहिए।

22. Skin Medicines की Storage Tips

अधिकांश topical medicines के लिए:

• पैक पर लिखी storage instructions follow करें।
• बहुत ज्यादा गर्मी से बचाएं।
• Direct sunlight से बचाएं।
• Tube/container अच्छी तरह बंद रखें।
• दूसरे व्यक्ति के साथ cream share न करें।
• Expiry के बाद उपयोग न करें।
• Medicine को original packaging में रखना बेहतर है।

विशेष formulation की storage requirements हमेशा उसके label/package insert के अनुसार देखें।

23. Pregnancy, Children और Elderly में Special Care

🤰 Pregnancy

कुछ topical और systemic medicines pregnancy में suitable नहीं होतीं। विशेष रूप से systemic retinoids जैसी medicines में बहुत महत्वपूर्ण safety restrictions हैं।

👶 Children

बच्चों में dose, formulation और application area अलग हो सकता है।

👴 Elderly

Polypharmacy, kidney/liver function और drug interactions पर विशेष ध्यान जरूरी हो सकता है।
